Even in the 21st Century, losing most or all of your teeth is still an unfortunate possibility. Many in this circumstance turn to dentures, as their great-grandparents did, to restore their teeth. But today's dentures are much different from those of past generations—and dental implants are a big reason why.
The basic denture is made of a gum-colored, acrylic base with artificial teeth attached. The base is precisely made to fit snugly and comfortably on the patient's individual gum and jaw structure, as the bony ridges of the gums provide the overall support for the denture.
Implants improve on this through two possible approaches. A removable denture can be fitted with a metal frame that firmly connects with implants embedded in the jaw. Alternatively, a denture can be permanently attached to implants with screws. Each way has its pros and cons, but both have two decided advantages over traditional dentures.
First, because implants rather than the gums provide their main support, implant-denture hybrids are often more secure and comfortable than traditional dentures. As a result, patients may enjoy greater confidence while eating or speaking wearing an implant-based denture.
They may also improve bone health rather than diminish it like standard dentures. This is because the forces generated when chewing and eating travel from the teeth to the jawbone and stimulate new bone cell growth to replace older cells. We lose this stimulation when we lose teeth, leading to slower bone cell replacement and eventually less overall bone volume.
Traditional dentures not only don't restore this stimulation, they can also accelerate bone loss as they rub against the bony ridges of the gums. Implants, on the other hand, can help slow or stop bone loss. The titanium in the imbedded post attracts bone cells, which then grow and adhere to the implant surface. Over time, this can increase the amount of bone attachment and help stymie any further loss.
An implant-supported denture is more expensive than a standard denture, but far less than replacing each individual tooth with an implant. If you want the affordability of dentures with the added benefits of implants, this option may be worth your consideration.

For generations, dentures have helped people avoid the dire consequences of total teeth loss. Now, implant technology is making them even better.
Composed of life-like prosthetic teeth fixed within a plastic or resin gum-colored base, dentures are manufactured to fit an individual patient’s mouth for maximum fit, comfort and performance. But dentures also have a critical drawback—they can’t stop bone loss in the jaw.
Bone is constantly regenerating as older cells dissolve and then are replaced by newer cells. In the jawbone, the forces generated when we chew travel through the teeth to the bone and help stimulate this new cell growth. When teeth are missing, though, the bone doesn’t receive this stimulus and may not regenerate at a healthy rate, resulting in gradual bone loss.
Dentures can’t transmit this chewing stimulus to the bone. In fact, the pressure they produce as they rest on top of the gums may actually accelerate bone loss. Over time then, a denture’s once secure and comfortable fit becomes loose.
In the past, most patients with loose dentures have had them relined with new dental material to improve fit, or have new dentures created to conform to the changed contours of the jaws. But implant technology now offers another alternative.
Implants are in essence a tooth root replacement. Dentists surgically implant a titanium metal post directly into the jawbone that naturally attracts bone cells to grow and adhere to it over time (a process called osseointegration). This not only creates a secure and lasting hold, it can also stop or even reverse bone loss.
Most people know implants as single tooth replacements with a porcelain crown attached to the titanium post. But a few strategically placed implants can also support either removable or fixed dentures. Removable dentures (also called overdentures) usually need only 3 or 4 implants on the top jaw and 2 on the bottom jaw for support through built-in connectors in the dentures that attach to the implants. A fixed bridge may require 4-6 implants to which they are permanently attached.
There are pros and cons for each of these options and they’re both more expensive than traditional dentures. In the long run, though, implant-supported dentures could be more beneficial for your bone health and hold their fit longer.
Many people with diabetes are hesitant about getting dental implants because they’re under the impression their chances of failure are greater than for non-diabetics. But if you’re one of the 26 million Americans with diabetes, that isn’t necessarily so — with a little extra precaution before, during and after implant surgery.
Diabetes is a group of diseases that affect how the body processes glucose. This simple sugar is used by the body to provide energy to cells, but can also cause damage if its volume level in the bloodstream is too high. The body normally regulates this through the hormone insulin produced by the pancreas.
The pancreas in people with Type 1 diabetes doesn’t produce insulin and so they must receive an outside source of the hormone through daily injections with careful daily monitoring of glucose levels. Those with Type 2 diabetes, the most common form of the disease, don’t produce a sufficient amount of insulin or the body no longer responds to the insulin produced. For either type, abnormal glucose levels — either too high or too low — can have adverse affects on the body, including blindness, nerve damage, gangrene, coma or death.
Diabetes can also slow wound healing, increase the risk of infection, and alter the body’s inflammatory response, all of which are major concerns when placing implants. Because implant placement involves minor surgery in which a wound results, there’s been wide concern that a slower healing process could increase the risk of implant failure.
Recent studies, though, are encouraging especially for patients who have their diabetes under control through medication, diet and exercise. Patients with poor glucose control are at higher risk, because it can take longer for the bone to heal around an implant after placement. For such individuals special considerations to guard against infection may be needed during implant surgery.
In fact, the implant success rate for most diabetics is about the same as for non-diabetic patients, 95%. With proper disease management and a little extra wound care, you can be among the many that experience a favorable outcome and a more attractive smile with dental implants.
Dental implants are considered the premier option for tooth replacement. While all implant procedures follow the same general concept — a titanium post surgically inserted into the jawbone with an attached life-like crown — the installation process can vary.
From their earliest history, implants have usually been installed through a two-stage process. In the first stage, the surgeon inserts the titanium post in the bone and leaves it “submerged” below the gum level to protect it from oral bacteria and the effects of chewing and biting. About three months later after the bone attaches to the titanium (a process called osseointegration), the surgeon then performs the second stage by re-exposing the implant and attaching a temporary abutment and crown for the patient to wear while the permanent abutment and crown are fabricated and later attached in 2-6 weeks.
In recent years, advancements in materials and design have made possible a one-stage process that allows the implant to protrude above the gum line during osseointegration and shortens the process. After the initial three-month healing period, the implant is ready for “loading” with the permanent crown.
The choice between which of these two procedures should be used for your implants will first depend on the type of tooth being replaced. A front tooth benefits from the one-stage procedure for cosmetic reasons because the surgeon can install a temporary crown to the exposed abutment during osseointegration (as long as the temporary tooth isn’t in functional contact with other teeth). An implant for a back tooth, on the other hand, doesn't have a large cosmetic demand so those one stage procedures usually end up with an exposed healing abutment but no temporary crown.
The strength of the bone is also a factor. Some bone tends to be softer, particularly in the back of the mouth. There’s a chance the implant could move in this softer bone, adversely affecting the outcome. For this reason, the two-stage procedure can be the preferred approach for posterior teeth as it offers more protection from movement.

Dental implants have become the standard for long-term tooth replacement. From mechanics to movie stars, people from all walks of life have discovered the advantages of replacing a missing tooth with an implant. Obviously, restoring your smile is a definite advantage, but an implant can also help to maintain the health of your jawbone and adjacent teeth.
The implant is a small, screw-like titanium post that is placed into your jawbone to function as the root part of the tooth. The living bone tissue will actually attach to the titanium post, fusing them together. This will not only provide a sturdy anchor for a natural-looking crown, but will provide stability for bridgework or dentures. At this point, if you are thinking that there must be a lot of pain involved, I have good news for you. There is very little pain involved after the procedure is completed and no pain at all during the procedure. Typically, it is a routine surgery that takes place in a dentist’s office under local anesthesia where the immediate area is numbed. If there is any apprehension at all about the procedure, we will offer alternative anesthesia or sedation options during the planning process.
To determine who will be a good candidate for the implant procedure, a plan must be in place to assure the success of the implant. Part of the plan includes:
- Reviewing your past medical and dental history. We must know your complete past and present medical history and medication use, since good health is essential. There are certain conditions and diseases that can affect the healing of an implant.
- Performing a comprehensive dental examination. An evaluation of your dental problems and needs will determine if implants are in your best interests. An assessment of the health and mass of the jawbone as well as the number and location of the implant(s) that are needed to restore your bite and smile back to health will also be determined.
When the implant procedure has been successfully completed, there is one more step. As with your natural teeth, preventive maintenance is crucial for long-term success. A daily routine of brushing and flossing along with regular professional dental cleanings and checkups will help ensure continued gum health and proper functioning of your implants.
